You will have access to our secure server with your user name and password.
Please provide us with a non-work/employment related e-mail address, and you can access your personal health record from a Smart Phone or any computer with an internet connection.
We will never share your email with any other entity. You will never receive health information through your email. Your email will be used to notify you that you have new information at your secure portal.

The Process
After you give us your email address we will activate your Portal Account. You will receive an email that provides you a link directly to the Patient Portal, along with a Log-in and Password.
When you arrive at the Portal you will use your assigned Log-in and Password to open the Portal. You will be asked an additional question to ensure your identity and then requested to change the assigned password to one of your choice.
Read and acknowledge the practice consent form and you are ready. These steps are necessary on your first log-in but will not be required on future log-ins.

Things You Can Do in the Portal
Inbox is where you will receive messages from the practice. Reminders, updates, education, etc. (At this time you cannot send messages to the practice).
Personal Information is where you can update your personal information. This should only be done in cases where there is an obvious error. For example, if we have your address as 215 Cedar Ave. and it should be 218, then you should make that change. However, don't make changes such Ave. to Avenue or Mary Jones to Mary J Jones. Electronic identifiers are picky and may not recognize you if these types of changes are made. If you have a question, call us for help.
Reset Password is used if you would like to change your password.
Intake Forms (Surgical and Allergies) is not activated at present so please do not use.
Lab/Diagnostic Reports are where you will find your results once they are released by the doctor. You will receive a notification through your email or via text message if you would like. Save time by not having to call for results.
Current Statement/Past Statement is for your convenience so you don't have to call the billing office.
Referrals will help you in avoiding being rescheduled because of a lack of the proper referral if required by your insurance carrier. Always check to make sure you have the proper referral before coming for your appointment.
Visit Summary provides you with an overview of your visit so you may actively participate in your care.
Current Appointment shows any pending appointments.
Historical Appointments provides you with previous appointment dates.
